We are seeking a Team Leader for our Primary Mental Health Service. The Wellbeing Service provides services to people who are experiencing mild to moderate mental health issues within a primary care setting. This varied role will lead the Well Being team and includes relationship management, service development, and some service delivery. This role will provide you with the potential to utilise your knowledge about mental health within a primary health care environment.

If you are a registered health professional with relevant experience and qualifications and enjoy working in a semi-autonomous capacity leading, planning and delivering client focused care this opportunity may appeal.

Outreach and Chronic Care RN

An exciting nursing position exists within our Primary Health Organisation. This person would work with Valley PHO practice nurses and doctors, and other PHO multidisciplinary staff to assist with meeting the health needs of those individuals who encounter barriers to access services or enjoying optimal health status. Promoting and assisting with “Care Plus” and Cardiovascular and Diabetes Risk assessment with Practices will also be a focus.

A Registered Nurse with significant experience in primary care and/or the management of chronic conditions would be ideal for this role.
